A toothache means a tooth has a problem, right? Most of the time, yes: the pain comes from a decayed or fractured tooth, or possibly a gum infection causing tooth sensitivity.

Sometimes, though, the pain doesn't originate with your teeth and gums. They're fine and healthy—it's something outside of your tooth causing the pain. We call this referred pain—one part of your body is sending or referring pain to another part, in this instance around your mouth.

There are various conditions that can create referred pain in the mouth, and various ways to treat them. That's why you should first find out the cause, which will indicate what treatment course to take.

Here are a few common non-dental causes for tooth pain.

Trigeminal Neuralgia. The trigeminal nerves situated on either side of the face have three large branches that extend throughout the face; the branch to the jaw allows you to feel sensation as you chew. When one of the nerve branches becomes inflamed, usually from a blood vessel or muscle spasm pressing on it, it can refer the pain to the jaw and seem like a toothache.

Temporomandibular Joint Disorder (TMD). These two joints that connect the lower jaw to the skull can sometimes become dysfunctional for a variety of reasons. This can set up a cycle of spasms and pain that can radiate throughout the jaw and its associated muscles. The pain can mimic a toothache, when it actually originates in the jaw joints.

Teeth Grinding. This is an unconscious habit, often occurring at night, in which people clench or grind their teeth together. Although quite common in children who tend to grow out of it, teeth grinding can continue into adulthood. The abnormally high biting forces from this habit can cause chipped, broken or loosened teeth. But it can also cause jaw pain, headaches and tenderness in the mouth that might feel like a toothache.

These and other conditions unrelated to dental disease can seem like a tooth problem, when they're actually something else. By understanding exactly why you're feeling pain, we can then focus on the true problem to bring relief to your life.

February is National Children's Dental Health month, sponsored annually by the American Dental Association. As important as good oral health is to a child's overall health and development, tooth decay tops the list as the most common chronic childhood disease. In fact, over 40% of children ages 2-11 have had cavities in their baby teeth.

If unchecked, tooth decay can have a profound impact on a child's quality of life. The good news is that tooth decay is preventable, and often reversible if detected early. Here are some things you can do to set your child on the path to good dental health for life:

Get your child in the habit of brushing and flossing every day. Cavity prevention starts at home, so teach your child to brush twice a day with fluoride toothpaste—but use only a smear of toothpaste the size of a grain of rice before age 3, and a pea-sized amount from ages 3-6. Introduce dental floss into the routine when you notice that your child's teeth are starting to fit closely together. Children generally need help brushing until age 6 or 7 and flossing until around age 10.

Encourage tooth-healthy eating habits. Provide your child with a balanced diet with plenty of vegetables, fruits and whole grains. Stay away from sugary snacks and beverages, especially between meals. If children drink juice, they should do so with meals rather than sipping juice throughout the day or at bedtime. Even 100% juice has natural sugars and can be acidic, which can harm teeth with prolonged exposure.

Establish a dental home early. Tooth decay isn't always easy to spot with the naked eye, so regular dental visits should start no later than a baby's first birthday. We can check the development of your child's teeth and spot any issues of concern. The earlier tooth decay is caught, the less damage it can do. Even if there are no dental problems, establishing a dental home early on will help your little one feel comfortable at the dental office.

Ask about preventive dental treatments. Fluoride varnishes or rinses are frequently recommended to help prevent cavities, particularly for children at higher risk of getting cavities. Dental sealants, another preventive treatment, are a coating commonly applied to molars to seal out tooth decay. According to the U.S. Centers for Disease Control, children ages 6-11 with dental sealants have nearly three times fewer cavities than children who do not have sealants.

Just what is sleep apnea?

Sleep apnea affects up to 20 percent of American adults, advises the National Sleep Foundation. What does this sleep disorder involve? Basically, people stop, or cease, breathing periodically when they sleep at night or nap during the day. They repeatedly awaken, gasping for air, and exhibit other symptoms such as:

  • Extremely loud and persistent snoring
  • Daytime sleepiness
  • Irritability
  • Depression
  • Poor concentration ("brain fog")

Besides these symptoms, sleep apnea has far reaching health effects such as diabetes, memory issues, dementia, high blood pressure and more.
Sleep apnea falls into three categories, namely:

  1. Obstructive Sleep Apnea, or OSA, where the soft tissues at the back of the throat relax and cover the airway as the person sleeps
  2. Central Sleep Apnea (CSA) in which the brain does not signal the respiratory system to take a breath
  3. Mixed Sleep Apnea, which is a combination of the above two

This dangerous sleep disorder happens more to men (by a ratio of two to one) than to women. Age (over 40) and being overweight factor into the diagnosis, too.


People who suspect they have this sleep disorder should speak to their primary care physicians who often refer to sleep physicians for definitive diagnoses by in-depth sleep studies.


A common treatment

Sleep physicians often prescribe CPAP, or Continuous Positive Airway Pressure, machines to combat sleep apnea, particularly when the diagnosis is CSA. A bedside CPAP machine is used at night to deliver a steady airflow through a nasal mask to the wearer. This air keeps the airway open, stopping most episodes of sleep apnea.
However, some people find CPAP therapy intolerable. They dislike the mask and the bulky, claustrophobic feeling it can create. The machines can be noisy, require diligent cleaning and present travel problems.

If there's anything that makes Alfonso Ribeiro happier than his long-running gig as host of America's Funniest Home Videos, it's the time he gets to spend with his family: his wife Angela, their two young sons, and Alfonso's teenaged daughter. As the proud dad told Dear Doctor–Dentistry & Oral Health magazine, "The best part of being a father is the smiles and the warmth you get from your children."

Because Alfonso and Angela want to make sure those little smiles stay healthy, they are careful to keep on top of their kids' oral health at home—and with regular checkups at the dental office. If you, too, want to help your children get on the road to good oral health, here are five tips:

  • Start off Right—Even before teeth emerge, gently wipe baby's gums with a clean, moist washcloth. When the first teeth appear, brush them with a tiny dab of fluoride on a soft-bristled toothbrush. Schedule an age-one dental visit for a complete evaluation, and to help your child get accustomed to the dental office.
  • Teach Them Well—When they're first learning how to take care of their teeth, most kids need a lot of help. Be patient as you demonstrate the proper way to brush and floss…over and over again. When they're ready, let them try it themselves—but keep an eye on their progress, and offer help when it's needed.
  • Watch What They Eat & Drink—Consuming foods high in sugar or starch may give kids momentary satisfaction…but these substances also feed the harmful bacteria that cause tooth decay. The same goes for sodas, juices and acidic drinks—the major sources of sugar in many children's diets. If you allow sugary snacks, limit them to around mealtimes—that gives the mouth a chance to recover its natural balance.
  • Keep Up the Good Work—That means brushing twice a day and flossing at least once a day, every single day. If motivation is an issue, encourage your kids by letting them pick out a special brush, toothpaste or floss. You can also give stickers, or use a chart to show progress and provide a reward after a certain period of time. And don't forget to give them a good example to follow!
  • Get Regular Dental Checkups—This applies to both kids and adults, but it's especially important during the years when they are rapidly growing! Timely treatment with sealants, topical fluoride applications or fillings can often help keep a small problem from turning into a major headache.

Bringing your kids to the dental office early—and regularly—is the best way to set them up for a lifetime of good checkups…even if they're a little nervous at first. Speaking of his youngest child, Alfonso Ribeiro said "I think the first time he was really frightened, but then the dentist made him feel better—and so since then, going back, it's actually a nice experience." Around 20 million people—mostly women after menopause—take medication to slow the progress of osteoporosis, a debilitating disease that weakens bones. But although effective, some osteoporosis drugs could pose dental issues related to the jawbones.

Osteoporosis causes the natural spaces that lie between the mineral content of bone to grow larger over time. This makes the bone weaker and unable to withstand forces it once could, which significantly increases the risk of fracture. A number of drugs have been developed over time that stop or slow this disease process.

Two of the most prominent osteoporosis drugs are alendronate, known also by its trade name Fosamax™, and denosumab or Prolia™. While originating from different drug families, alendronate and denosumab work in a similar way by destroying specialized bone cells called osteoclasts that break down worn out bone and help dissolve it. By reducing the number of these cells, more of the older bone that would have been phased out lasts longer.

In actuality this only offers a short-term benefit in controlling osteoporosis. The older bone isn’t renewed but only preserved, and will eventually become fragile and more prone to fracture. After several years the tide turns negatively for the bone’s overall health. It’s also possible, although rare, that the bone simply dies in a condition called osteonecrosis.

The jawbones are especially susceptible to osteonecrosis. Forces generated by chewing normally help stimulate jawbone growth, but the medications in question can inhibit that stimulus. As a result the jawbone can diminish and weaken, making eventual tooth loss a real possibility.

Osteonecrosis is most often triggered by trauma or invasive dental procedures like tooth extractions or oral surgery. For this reason if you’re taking either alendronate and denosumab and are about to undergo a dental procedure other than routine cleaning, filling or crown-work, you should speak to your physician about suspending your medication temporarily. Dentists often recommend a suspension of three to nine months before the procedure and three months afterward.

Some research indicates this won’t worsen your osteoporosis symptoms, especially if you substitute another treatment or fortify your skeletal system with calcium and vitamin D supplements. But taking this temporary measure could help protect your teeth in the long run.
